Insurance

Most private insurance plans are accepted. Please call our office or your insurance carrier for verification.

NO INSURANCE? NO PROBLEM!

Our Certified Application Counselors can help you determine if you qualify for insurance programs such as Medicaid, CHIP, CHIP Perinatal, or the Marketplace insurance plans during active enrollment periods. We also offer a sliding fee discount program based on household size and income. Contact us for more information.

Sliding Fee Discount (Qualify for $10 Visits)

Sliding fee discounts are available to those who qualify. Specific conditions must be met, and all appropriate documents must be submitted. Sliding fee calculations are based on income and size of household. If you do not qualify for the sliding fee discount or do not have all documentation, you will be responsible for paying full price. We accept cash, check and credit cards.

Good Faith Estimate / No Surprises Act

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate beforeyou schedule an item or service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate.

Getting Ready for Your Appointment

New patients are advised to show up at least 30 minutes before your appointment time to allow enough time to register, sign the paperwork, verify insurance coverage, and sign up for the patient portal. Existing patients are advised to show up at least 15 minutes before your appointment time to validate existing patient information, verify insurance coverage, and sign paperwork. You will have the option to receive an appointment reminder via phone, text, or email. Please remember to confirm your appointment when notified.

All patients should bring the following items: driver’s license or other photo identification, current health insurance card, and current medications. To determine if you are eligible for a discount on services, please bring proof of income and household size.
