All new patients must download the Patient Portal Registration and Consent Form located in the Patient Registration Packet. Bring this form in to your appointment and complete in person with staff. Visit our customer service desk at any of our locations and a Foremost staff member will assist you in completing the patient portal registration. The patient portal provides you with 24/7 communications with your provider, the health team and the clinic. Foremost Family Health Centers will provide ongoing education on the use of the patient portal which enables patients to:
Communicate with our practice, securely and efficiently
Email and secure messaging for non-urgent needs
View some of your Personal Health Records
Review some laboratory results
View financial statement
Request an appointment, and see the date and time of upcoming appointments
Request prescription refills (no controlled substance)
Update your demographic information (i.e. address, phone number, insurance)
Manage personal information in our database and request information
No sensitive subject matters will not be provided over the portal (HIV, mental health, work excuses, etc.). All communication via the patient portal is private, secure and will be included in your permanent patient record.
The Patient Portal is NOT for Urgent Communications. Foremost Family Health Centers will maintain and manage patient portal requests during our normal business hours. Please allow 48 hours to 3 business days for a reply.
If your need is urgent please call us at (214) 426-3645 during office hours and (214) 360-2963 after-hours. For life-threatening conditions, call 911 or go to the nearest emergency room. All portal functions may not be available to you. Function availability will be based on guidelines from HIPAA, and laws regarding protected health information.
